>> This page contains the benefits of listing your practice in the listing. Click on the "SIGN UP NOW!" link at the top or bottom of this page.
>> Fill in the required fields in the Add Entry Form

>> Click the "Fetch Coordinates" button half way down the page. This will fill in the longitude and latitude fields so that the google map functions will work.
>> If uploading an image or logo click on the "Choose File" button and select the image file on your computer.
>>Enter relevant keywords in the "Visible Keywords" and "Meta Keywords" for search engines to find you.
>>Enter one or two sentences in the "Meta Description" which may be displayed in the search results page of search engines.
>> In the Please Select Category section click on your state and then click on the " >>>" button. This should move your state into the small box on the right.

| >> Scroll down and click on the PayPal image with credit cards |

Now you have 3 options.
1) Simply pay with credit card using the form on this page ($350 annual payment only)
2) Sign in and use your paypal account. Be aware, if you sign in you will have to use the credit cards or checking account already on file. ($350 annual payment only)
3) Call us at (870) 576-9635 and we'll take your credit card over the phone. ($350 annual or $39.95 monthly payment)

You will automatically receive an e-mail stating your entry has been added and asking you to pay with PayPal. Please check your junk e-mail folder as our automated e-mail often gets filtered as spam If you have already paid with PayPal or on the phone you can ignore this e-mail.
